Based on the “Gabriel Case Scenario,” use the “Transition Goals Template” to draft one measurable transition goal for Gabriel in each of the following areas: 1. Education/Training (i.e., vocational, post-secondary education, on-the-job training, apprenticeship) 2. Employment (i.e., sheltered, volunteer, competitive) 3. Independent Living (i.e., daily living skills, leisure, transportation, access to adult services) For each transition goal, identify one applicable transition-related service and one activity appropriate for Gabriel to reach his goals. Beneath the template, in 250-500 words, write a rationale explaining the goals and activities you planned for Gabriel and how they demonstrate compassion, justice, and care. Support your findings with a minimum of two scholarly resources. This assignment uses a rubric. Review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion. ADDITIONAL PROFESSOR NOTES: Please read carefully because this is a transition plan and not an IEP. You have a transition template provided that includes your goal, the services that will support your goal and the activities. The goal is what you want Gabriel to master by the time he graduates while the services and activities are what the special education team will provide to help Gabriel reach those goals.
